Who is eligible for Gas Reimbursement?
Priority Health provides gas reimbursement to Medicaid members and other eligible individuals providing transportation for Medicaid covered medical services. To be eligible for gas reimbursement, the Medicaid member must be in the vehicle. Gas is reimbursed at the level of $0.63 per mile. Reimbursement is calculated based on the most direct route from your home to your medical provider as generated by our mapping software. Payments will be distributed electronically, unless you specify otherwise.

How do I sign up as a driver?
To sign up as a driver, please submit your information using our Driver Registration form. We will need your contact information, a photo of your driver’s license and vehicle insurance (if you have a smartphone, you can use the camera on your phone), and your payment information.
Does the driver need to own the vehicle?
No, the driver does not need to own the vehicle, but the driver does need to provide valid vehicle insurance.
What if some of my information changes?
To update your contact information, driver’s license, vehicle insurance, or banking information, please resubmit your application to be a driver using the Driver Registration form. Your new information will be automatically updated on your profile.
When should I call you about my trip?
You can call for gas reimbursement the same day of your appointment as long as you call before your appointment.
When should I submit the gas reimbursement form?
Please submit the form after you have completed the trip.
Can I fax in the Gas Reimbursement Driver Registration form?
Yes, you can print and download our Gas Reimbursement Driver Registration from our Forms page. Please fax completed forms to 1-855-667-2557.
When will the reimbursement be sent out?
If you’ve chosen to receive payment electronically, you should receive payment within 1-2 weeks. If you’ve chosen to be paid via check, you should receive payment within 2-4 weeks.
Routine medical appointment hours are Monday through Friday 8 am to 5 pm EST. For routine appointments, you can call for gas reimbursement the same day of your appointment, as long as you call before your appointment.
Urgent and same day appointments are accommodated. If you have an emergency transportation need, please call 9-1-1 immediately.
